Our new client form

Accelerate your treatment and save time by helping us understand you before you attend.

The information provided in our time saver form can save you up to 60 minutes of history taking and assessment time  – we want to save you time and costs by getting to the problem solving and hands on treatment / rehab phases quicker.

Please complete your new client time saver form 24-48 hours before your first session with us by using one of the following two options:

Option 1 - electronic form (preferred)

  • Simply click the link below and complete the form on your phone or electronic device.
  • When finished click submit and your information will be instantly in your file.
  • If time allows you will receive a pre-consultation call from your Physiotherapist to further discuss your injury, condition or goals

If you experience difficulty completing this form electronically, please contact admin to ensure the document can be completed.

Option 2 - word document form

  • Download, read and complete the new client form below.
  • Return via email to info@centralphysiohealth.com.au
  • If time allows you will receive a pre-consultation call from your therapist to further discuss your injury, condition or goals. 

Please note, this document is not compatible with some electronic devices. If you experience difficulty completing this form electronically, please arrive 15-20 mins prior to your session to ensure the document can be completed.

Your agreement to us

Non-attendance: A minimum of 24 hours’ notice is required for appointment changes or cancellations. This allows other clients requiring treatment to take the cancelled appointment with your therapist. Appointments cancelled, changed or not attended with less than 24 hours’ notice will be charged at full fee rate for the time booked with your therapist that another client can no longer use.

Upfront payment: We request that all accounts are paid for directly after the treatment is conducted.

Privacy policy: By signing our new client terms and conditions, you confirm that you agree with the terms of our Privacy Policy, including the collection, storage and permitted disclosure of your personal information and health information as set out in our Privacy Policy. A full copy of our Privacy Policy is available in our footer should you wish to read it in detail.

We want you to trust that we value your time as much as you value ours... If your treatment session is cancelled you must be given 24 hours’ notice. Failure to do so will result in you being offered the same time with another therapist or a complimentary session.

Reminder SMS: If you provide a mobile contact number a courtesy SMS reminder will be sent the day prior to your appointment. There are times when we are unable to contact you, therefore do not rely on these SMS messages.

Privacy policy: We agree to adhere to the terms and conditions stated in our Privacy Policy in the protection and storage of your personal and health information, and agree to only disclose your personal or health information where authorised by you in accordance with our Privacy Policy.
